Community Central has an exciting opportunity available for an experienced health care professional – 0.8 FTE
Community Central is a triage service comprised of a multidisciplinary team of both Nursing, Allied Health clinicians and Administration, who are rostered across seven days between the hours of 0800-1800, 8hr shifts.
The aim of Community Central is to provide a single point of contact ensuring a seamless, visible and safe transition of care for all patients from referral, to service provision. The focus is on customer service and patient experience, ensuring any additional needs/information is captured, which enables the best possible outcomes for both the patient and Whaanau.
You will be working with a dynamic and supportive team committed to the delivery of health care excellence and living the Counties Manakau Health values of Together, Kind, Excellence and Valuing Everyone. The role will involve developing strong networks with relevant key stakeholders, as well as understanding the cultural diversity, resources and specific health issues of the Counties Manukau Health demographic.
Essential
- NZ registered health care professional with current annual practising certificate
- Minimum 3-5 years relevant experience post qualification
- Knowledge of local community settings
- Excellent communication skills
- Sound computer literacy skills
- Full, clean driver’s licence
Desired
- Evidence of post graduate study
- Advanced assessment skills
- Experience in both hospital and community setting
- Needs Assessment experience
