Kōrero mō te tūranga – About the role

This role is part time 0.6 (3 days a week) it is fixed term, covering an Specialty Clinical Nurse (SCN) vacancy for a maternity leave cover secondment. We are looking for a dynamic and motivated nurse to join our specialised team of senior nurses. Rheumatology or Long term Conditions experience would be of benefit, likewise experience in conducting nurse led clinics. You will be experienced in working as part of a busy team to ensure patients are well supported.

Mō tō mātou rōpū – About our team

Our role is based in the Wellington Regional Rheumatology Unit at Hutt Hospital, with clinics held across the sub region at Hutt Hospital, Wellington Hospital, Kenepuru Hospital and in Greytown. We have a team of senior nurses who work alongside a wider clinical team of SMO-s and a Registrar. Our nurses are an integral part of the service, providing Nurse led and combined clinics, as well as patient contact and support. We have a strong commitment to equity and have many upcoming change projects to further improve our service. We approach change as a team, our whole service being involved in growing and improving service provision.

Moū – About you

As our new team member you will:

  • Provide effective nursing assessment, care planning and evaluation of patients * Provide education and support to patients to improve self-management * Work with other members of the team in identifying, planning and implementing positive changes to models of practice * Effectively collaborate and communicate with the interdisciplinary team to support enhanced health outcomes for patients * Contribute to and maintain evidence based practice and research to support high quality, safe and effective patient care * Actively contribute to our work to achieve equity of health outcomes for Māori, Pacific peoples, and disabled people.Some of the clinics for this role are at our outreach clinics, as such you will need to be open to travelling within the region. At least one of your regular weekly clinics will be based at Wellington or Keneperu hospital, however your base will be at Hutt Hospital.

SKILLS AND EXPERIENCE

  • Senior Nurse or experienced outpatient RN * Experience in Rheumatology or other chronic/pain conditions desired * Strong communication skills necessary * Experience in peer education * Confident working autonomously as well as in a team * Initiative and a can do attitude * Full Drivers licenceE pā ana ki a tatou – About us
