Kōrero mō te tūranga – About the role
The community nurse provides a holistic case management model for children and young people in the community setting with a strong focus on empowerment supporting whanau to manage their child-s health condition. This covers a wide geographical area between Kenepuru and Wellington. This is not limited to the home and may take place in the outpatient setting, primary healthcare practices or schools. The role encompasses a range of nursing assessments, interventions and evaluations for acute and long term conditions. The role includes inpatient support and consultation facilitating complex patient discharges. The role works alongside other agencies and interdisciplinary teams to manage children and young people with high health needs and health disparities. Post graduate study is essential and progression to expert on the PDRP. A willingness to be innovative and contribute to the ongoing development of the service is essential.
Duties
