We are currently seeking a suitably health and allied professionals (with a current practising certificate) join Community Health Integration Centre (CHIC) Team. Established in 2019, the CHIC team is busier than ever, and we need to grow our team.

The Community Health Integration Centre (CHIC) provide a holistic patient clinical assessment and triage service for the Taranaki population, currently across 16 services with more coming on stream in early 2022.

Clinical Assessors ensure that patients are assessed, triaged and connected to receive timely health care in the most appropriate setting They are responsible for ensuring quality services. There is a strong emphasis on developing collaborative relationships with internal stakeholders and partners, with the local communities, health and social agencies to strengthen the support for patients and their family/whanau.

Expert clinical judgement and knowledge of the Taranaki health landscape is essential as is a proactive approach to problem solving and achieving equitable patient centered solutions.

We are looking for health practitioners who understands the value of inter-professional working across different health professions and are committed to providing the highest levels of clinical care.
