About the team
The Home Team is an interprofessional team currently consisting of Occupational Therapists, Physiotherapists, Registered Nurses, and Rehabilitation Assistants. Their interprofessional practice is underpinned by the Calderdale Framework to support skill sharing among the professions.
The Home Team provides a hospital avoidance, early supported discharge service including a community rapid response service. Whilst they cater for all ages above 16 years, their work is predominantly with older persons or those who are close in age and interest.
About the Opportunity
The role will involve working alongside our existing Older Persons Health Community Nurse Practitioner and the Home Team. The Community Nurse Practitioner provides comprehensive assessment and plans of care for our Older People in our community, including supporting Aged Residential Care. The primary purpose of this position is to promote partnership and integration across primary, community and secondary services for the Home Team and OPH.
This role will see you develop and influence education, health policies, clinical standards/practice, and service development at a local level with a quality improvement focus. You will have the opportunity to provide advanced nursing care for people with dementia, delirium, and those older person’s presenting with significant challenging behaviour, through the development of a collaborative/interprofessional model of care with the interdisciplinary team.
About you
This is a demanding and challenging role where the successful applicant will demonstrate sound professional judgement, pursue clinical excellence, strong clinical leadership skills, a clear commitment to high quality care and improved patient outcomes and contribute to the development of the Service.
A key driver in your success will be the relationships that you form with your team and key stakeholders. You will need to be able to think outside of the square and communicate and influence across a range of providers.
To succeed in this exciting role, you will also need to:
- Be a Registered Nurse Practitioner with a valid Annual Practicing Certificate or a Registered Nurse on the Nurse Practitioner Pathway with an expected completion date of 2 years
- Minimum of 5 years’ experience in Older Persons Health or Primary Care and ideally acute care
- Have the ability to successfully engage stakeholders in a positive manner
- Have demonstrated knowledge and application of NZ Health strategies
- Be a fabulous communicator with an engaging and personable style
This is a very rare opportunity for a Registered Nurse Practitioner or Registered Nurse who is on the Nurse Practitioner pathway. If on the Nurse Practitioner pathway, it is expected this would be completed within 2 years while working as part of our Home Team and Older Persons Health Team.
