We have an exciting opportunity to join the Whānau & Communities Group as Project Manager in particular to coordinate and develop the Long Term Conditions model of care in conjunction with the Nurse Practitioner LTC Lead. Initial focus is on the Integrated Respiratory Partnerships and Pathways for whanau.
It is an exciting time to be engaged in the health sector and this is a great opportunity to become part of a dynamic and supportive team who are passionate about improving services for our whānau in their local communities. You will work closely with Whānau and Communities leadership team, and key stakeholders.
