In collaboration with WellSouth PHO, we have implemented an exciting initiative providing additional support in General Practices with the aim of improving health outcomes and overall wellbeing.
The Role
This is a combined role, working as both a Health Coach and Community Support Worker in the Access and Choice service. The primary purpose of the Access and Choice service is to work in collaboration with a General Practice team to assist patients with health challenges to improve their general wellbeing.
This role has a service development component and is an exciting opportunity to be part of a positive initiative in primary health.
In the Health Coach aspect of the role, you would be based in the general practice, helping patients to self-identify goals to improve their overall wellbeing. This may involve social, health, education, whanau and cultural reconnection. You will be motivating people to make positive lifestyle and wellbeing changes, as well as finding tools for managing stress and reducing high risk behaviours. The role will involve working with patients individually in 30 minute appointments and may involve some group sessions.
In the Support Worker aspect of the role, you will be based in the community and will journey alongside individuals as they work towards achieving their self-identified goals. You will assist individuals in implementing the plans and goals as developed in the health coach role, helping to remove any barriers along the way. In this role you will need to develop and maintain strong relationships with external stakeholders in the community.
This dual role provides an opportunity to be an active participant in all aspects of Access and Choice patient support.
The hours of work are at present 24 per week, worked between Monday – Friday.
