Emerge Aotearoa is a social services organisation providing a range of services to whānau, including Health and Disability, Youth and Transitional Housing. Our kaimahi all have a common goal – helping people realise their potential with the aim of supporting whānau to live their best life as independently as possible.

Te Tumu Waiora – te reo for ‘to head towards wellness’ – is a new model of primary mental health and addictions care and support which aims to provide all New Zealanders experiencing mental distress or addictions challenges with access to convenient, high quality, integrated and person-centred care and support. The model is being rolled out nationally over the next four years.

Te tūnga | The Role

Community Support Worker’s with Access and Choice aim to connect directly with whaiora who are referred via their GP practice. This involves working alongside whaiora at their GP practice to develop and complete identified goals for up to three months to improve their health and wellbeing. For example, navigating social services, developing personal plans, gaining employment, budgeting, and housing. You will also be an integral part of the general practice team and working in collaboration with other community partners, as well as building new relationships through networking.

The ideal person will need to have strong communication skills including the ability to network and maintain collaborate relationships, manage caseloads and have good computer skills. An understanding of primary care services and a level 4 qualification in health would also be advantageous. Further role specific training will be provided to the successful applicant through orientation and beyond!

While you will work to support patients of all ages, you’ll predominately support people aged between 18 and 70.

This will be covering the Wellington and Lower Hutt region is a Full-time vacancy working 40 hours per week on Monday – Friday.

Māu | About You

To be successful in this role, you’ll have a level 4 or 5 qualification in Mental Health and Addiction. You’ll have the ability to build relationships and rapport easily, while maintaining professional boundaries.

You’ll need to be a strong effective communicator and have the ability to have patients open up in a limited timeframe. Our patients also come from an array of backgrounds so cultural competency and the ability to connect to people from different backgrounds and cultures are essential to this role.

You’ll need to meet the Children’s Act safety checking and our vetting requirements and have a full clean, valid driver’s licence. You’ll have strong computer skills and feel comfortable completing case notes.
