The Team: What

we do

Our Adult

Community Referral Centre (ACRC) within the Canterbury District Health Board is

a diverse professional team based at The Princess Margaret Hospital

ACRC is the

main point of contact for Older Persons Health & Rehabilitation, and is

responsible for triaging all referrals coming into our community based teams

covering Older Persons physical and mental health, post stroke community

rehabilitation and adult community therapy service. ACRC receive, triage, and

allocate about 2000 referrals per month. This busy team is pivotal to the

successful operation of the Older Person-s Health interdisciplinary community

teams.

The Role:

Where you fit in

In this role, you

will be responsible for:

prioritising, organising and managing incoming referrals with an Occupational

Therapy lens. * Providing

excellent customer telephone service, identifying the needs of the caller and

provide appropriate assistance, problem solving in handling enquiries, and

forwarding on appropriately to other team members. * Participating

in quality initiatives and identify areas of quality improvement.Your skills will

include:

Occupational Therapy Board of NZ and hold a current Practicing Certificate * A thorough knowledge base

of community and home-based support services and resources * The ability to build and

nurture relationships with service providers in the community * Excellent communication

and listening skills including a high standard of courtesy and interpersonal

skills. * Proficient Information

Technology skills – experience in Patient Management Systems,

spread-sheets and research applications will be an advantage. * An ability to think on

your feet, problem solve, and be an adaptive learner. * An ability to work and

relate well within a team, excellent time management, and ability to

manage stress in a positive manner.Our Culture:
